Position: Court Support Worker

QUALIFICATIONS:

Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the CNO. (Preference to those with Canadian Certification in Psychiatric/Mental Health Nursing); Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or Bachelor’s Degree with an applicable college diploma in Addictions and current registration with the C.A.C.C.F /or Grandfathered Post-Secondary Degree.

SALARY RANGE: Case Manager/RPN Grid

HOURS OF WORK: 35 hours per week

STARTING DATE: As soon as possible

This position is responsible for providing person-centred, recovery oriented, short term case management to facilitate the discharge planning process for individuals with symptoms of or diagnosis of mental illness who are currently in custody and who require assistance reintegrating into the community. Once released from custody / court support, support services will assist individuals to navigate the just system, link to required mental health supports and treatment, advocate for services and entitlements and coordinate information sharing for case planning and act as a mental health resource.

A key role of these positions is to establish partnerships with the key areas of the justice system such as South-Western detention centre, probation and parole, Chatham Kent Police, Court Delegates (i.e. lawyers, court officers, etc.) and Chatham Kent Health Alliance.

Reporting to the Manager of Client Services, the incumbent will be responsible for the following functions:

  • Engaging individuals at their initial point of contact with the Criminal Justice system.
  • Completing the referral and assessment process to determine individual needs and services.
  • Acting as a resource to CMHA staff and other service providers in the Sarnia Lambton.
  • Liaising with correctional facilities to provide assessments in preparation for discharge.
  • Assessing and determining eligibility for Court Diversion.
  • Serve as access and coordination points for the service and criminal justice systems.
  • Develop linkages and/or protocols with service providers to ensure continuity of care and reduce risk of future recidivism.
  • Provide assertive outreach to treatment-resistant individuals though the period of their court matters.

The candidate will work with other professionals using strong problem solving and communication skills. Demonstrated ability to build partnerships with community partners is required.

A valid driver’s license, and use of a vehicle, which will be used in conducting agency business, is required.
