Program Summary:
Mount Dennis ICM provides specialized care coordination and one-on-one support for adults and seniors with mental health and addiction challenges. Our Case Managers support clients by facilitating recovery. Our program also supports the entire building of Toronto Community Housing tenants with community development engagement and activities. These can range from bringing in health education and resources to celebrating holidays with special meals and events. The goal is to create a vibrant, healthy community within the entire building.
Job Summary:
As aCase Manager you will work independently or be part of a team that co-ordinates a support network and links clients living with mental illnesses, addictions and physical health issues. The position attempts to empower the client, thereby increasing their independence, dignity and self-respect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.
