Program Summary:
McEwan Housing and Support Services offers 10 high-support residential units, and 4 respite units for HIV+ or transgender individuals experiencing mental health and/or addiction challenges. We have Personal Support Workers (PSWs) who assist with daily activities and life skills development to promote independence. PSWs also support chronic care and palliative needs. McEwan clients and former clients are members of the General Members Association (GMA). The GMA provides a forum for safe community building and interaction. Members actively engage in monthly social and recreational events. The program operates inside two semidetached homes, with 3 floors located in downtown Toronto. One of the homes is wheelchair accessible and has access to chair lift inside the home and a ramp outside the home. McEwan Housing and Support services offers 24/7 services directly to residents living in the home or to respites on-site. The McEwan Housing & Support Services Program follows the principles of Psychosocial Rehabilitation, which has as its goal, empowerment, for the people we support. The Respite Program offers care to members of the community that live with HIV and require support around the clock to accomplish a specific outcome. The Respites are goal-oriented, related to health and mental health recovery. Respites provide access to the high support residence at no cost to ensure housing is maintained, health improved as well as goal planning and length of stay is determined by need and space availability.
The goal of the McEwan House Residential program is to house and support those who are unable to manage independently in the community. A Plan of Care is created to meet individual needs prior to moving into McEwan. The adjoining houses at 20 and 22 Dundonald Street provide 24-hour on-site support. In addition, there are Personal Support Workers on-site seven days a week for extended hours, to assist residents with cooking, cleaning, laundry, hygiene, meal preparation and attending appointments. Residents have 24-hour access to staff by phone. For overnight or weekend staff that may be responding to member calls, there is also 24-hour coverage through on-call staff support. Residents are encouraged to support one another and assume an active role in the management of their home. When residents are not able to work out interpersonal issues between themselves, they are encouraged by staff to utilize house meetings, individual support sessions or conflict mediation.
Job Summary:
The coordinator provides overall team leadership and ensures the effective delivery of harm reduction and outreach services across the regions. This role oversees day-to-day operations, including Case Manager services, and supports clients by facilitating optimal care through a range of assistance and resources. In collaboration with the Manager, the Coordinator develops and maintains relationships with network partners, represents LOFT at system tables, and ensures staff including partner agency staff are informed about program operations and documentation requirements for reporting purposes. The coordinator is also responsible for guiding staff, maintaining service quality, and fostering strong relationships with individuals who are unhoused, at risk of homelessness, street-involved, or experiencing mental health and substance use challenges.
