Job Description:
The Inpatient Palliative Care Unit at Trillium Health Partners (THP), located at the Mississauga Hospital (MH) site, consists of 16 beds dedicated to providing specialized palliative and end-of-life care to patients with life-limiting illnesses.
Working in close collaboration with palliative care physicians and an interprofessional team, the Social Worker is a highly skilled, adaptable clinician with strong critical thinking and communication abilities. In this role, you will provide patient- and family-centred care focused on optimizing comfort, function, and quality of life. Care delivery emphasizes symptom management, maintenance of mobility and dignity, and support for patients and families navigating complex and sensitive stages of illness.
Recognizing the unique needs of this population, care is guided by patient preferences, functional tolerance, and evolving goals of care. Family involvement and education are integral components of care delivery.
Reporting to the Clinical Manager, the ideal candidate is committed to delivering exceptional, compassionate care in a dynamic and emotionally complex environment. This role offers the opportunity to practice with empathy, clinical excellence, and professionalism while supporting patients and families through end-of-life care and decision-making.
Key Qualifications:

Masters of Social Work degree with experience in individual, couple, group and family counselling, case management and community resources.

Membership in good standing with the Ontario College of Social Workers and Social Services Workers (OCSWSSW) and the Ontario Association of Social Workers (OASW).

Candidates with recent clinical experience, knowledge, education, or certification in palliative care preferred

Understanding and experience working in a community and an outpatient support.

Effective interpersonal, organizational and communication skills
-
Excellent interpersonal, communication and organizational skills.
-
Interest/experience in the integration of social work care, research and education.
-
Reliable and flexible individual to work as a member of an interprofessional team.

Experience in providing education to patients and families in individual and group settings
-
Experience with end of life care is preferred

Satisfactory performance and attendance.
Key Responsibilities:

Comprehensive psychosocial assessment and focused consultations
-
Assessing the psychosocial needs; develop and formulate goal oriented social work treatment plan and interventions including discharge planning and arrangements, provide patient/family education and support to address psychosocial, emotional and financial needs.

Short term interventions and/or referral and linkage consultation with relevant providers or agencies.

Ongoing case management within the mandate of the team.

Telephone support and counselling.

Consultation, advocacy and education of clients/families to meet the complex needs of patients and families.

Responsible and accountable for decisions and actions in the provision and management of patient care within the scope of practice of social work as defined by OCSWSSW as a collaborative member of the interprofessional team.

Participates as a member on relevant internal and external committees and professional groups.
-
Collaborates with other members of the interprofessional team to effectively plan services.
-
Contributes to the development of team rounds, clinical conferences, and team meetings.

Provides education to students and health professionals.

Participates in quality management, evaluation and improvement initiatives within social work and teams, programs and/or services.
