Position Summary

The Registered Practical Nurse (RPN) provides safe, competent, and compassionate nursing care to clients in accordance with the standards of the College of Nurses of Ontario (CNO), employer policies, and applicable legislation.

Working collaboratively with Registered Nurses (RNs), physicians, Personal Support Workers (PSWs), and other healthcare professionals, the RPN is responsible for delivering direct nursing care, monitoring client health, administering medications and treatments, and promoting the highest quality of life for every client.

Assignments may include private homes, retirement residences, long-term care facilities, community programs, and other healthcare settings.

Primary Responsibilities

Clinical Nursing Care

  • Provide safe and effective nursing care to clients.
  • Perform comprehensive and ongoing client assessments.
  • Monitor vital signs and changes in health status.
  • Identify health concerns and report significant findings promptly.
  • Develop and implement individualized care interventions within the RPN scope of practice.

Medication Administration

  • Safely administer prescribed medications.
  • Monitor medication effectiveness and adverse reactions.
  • Maintain accurate Medication Administration Records (MAR).
  • Educate clients regarding medications and treatment plans.

Treatments & Procedures

  • Perform wound care and dressing changes.
  • Administer injections as authorized.
  • Manage catheter care and ostomy care.
  • Monitor blood glucose levels and diabetic care.
  • Assist with oxygen therapy and respiratory treatments.
  • Perform other nursing procedures consistent with RPN scope of practice.

Care Planning

  • Participate in developing individualized care plans.
  • Review and update client care plans regularly.
  • Collaborate with interdisciplinary healthcare teams.
  • Promote client independence whenever possible.

Supervision & Collaboration

  • Work collaboratively with Registered Nurses, physicians, therapists, and allied health professionals.
  • Provide guidance and support to PSWs.
  • Delegate tasks appropriately within legislation and organizational policies.
  • Participate in team meetings and care conferences.

Client & Family Education

  • Educate clients and families regarding:

  • Healthy lifestyle practices
  • Promote informed decision-making.

Documentation

Maintain accurate and timely documentation, including:

  • Nursing assessments
  • Progress notes
  • Medication records
  • Incident reports
  • Care plan updates
  • Client communications

Ensure all documentation complies with regulatory standards and Averra Care policies.

Infection Prevention & Safety

  • Follow all Infection Prevention and Control (IPAC) protocols.
  • Utilize appropriate Personal Protective Equipment (PPE).
  • Adhere to workplace health and safety standards.
  • Maintain a safe environment for clients and staff.

Quality Improvement

  • Participate in quality improvement initiatives.
  • Support accreditation and compliance activities.
  • Assist with audits and clinical reviews.
  • Contribute to best practices in client care.

Qualifications

Required

  • Diploma in Practical Nursing from an accredited institution.
  • Current registration and good standing with the College of Nurses of Ontario (CNO).
  • Current CPR (Level C) and First Aid Certification.
  • Valid Vulnerable Sector Check.
  • Current immunization records and TB screening.
  • Ability to travel between client locations (community assignments).

Preferred

  • One (1) year of nursing experience.
  • Experience in:

  • Experience working with seniors and individuals with complex health conditions.
  • Wound care certification is considered an asset.
  • Palliative care experience is an asset.
  • Dementia care training is an asset.

Physical Requirements

The successful candidate must be able to:

  • Stand and walk for extended periods.
  • Assist clients with transfers and mobility.
  • Lift, push, or pull up to 50 lbs using safe body mechanics and assistive devices.
