This opportunity is with our Good Shepherd – Seniors at Risk Program
Good Shepherd Centres Hamilton, a ministry of the Hospitaller Order of St. John of God, strives to enhance the quality of life for all who seek assistance and support in our community. The Seniors at Risk in Hamilton (SaRiH) program supports vulnerable seniors by helping them access essential services, resources, and community supports while promoting health, safety, wellness, and independence.
How You Will Make an Impact
In this role, you will provide individualized home and community-based support to vulnerable seniors and work collaboratively with clients, care teams, and community partners. You will help clients identify their needs and goals, develop care plans, access community resources, and overcome barriers to care.
Services include case management, referrals, advocacy, system navigation, crisis prevention and intervention, accompaniment to appointments and community activities, and support with activities of daily living. The successful candidate will demonstrate a commitment to providing services in a dignified, culturally competent, anti-oppressive, and respectful manner.
Responsible for participating in Good Shepherd's Health and Safety programs, procedures, procedures and work in compliance with the provisions of the Occupational Health and Safety Act and its regulations to minimize risk to clients, staff and others as well as to the property of Good Shepherd.
