High-Level Overview for Candidates

TAPP Workforce Solutions is recruiting a Primary Care Nurse Practitioner on behalf of a community health organization serving diverse populations in Scarborough. This role focuses on delivering comprehensive, family-centered primary care to unattached patients, with an emphasis on health promotion, disease prevention, and equitable access to care.

Key Responsibilities

Provide comprehensive primary care across the lifespan within NP scope
Assess, diagnose, and manage acute and chronic conditions
Deliver preventive care, including immunizations
Develop and implement patient-centered care plans
Collaborate within an interdisciplinary team
Support marginalized populations with complex health needs
Participate in health promotion and outreach initiatives
Maintain clinical documentation and ensure confidentiality
Contribute to quality improvement initiatives
Participate in on-call rotation and occasional home visits

Requirements

Education

Master’s degree in Nursing

Certifications

Primary Health Care Nurse Practitioner (PHCNP) certification
Registered in good standing with the College of Nurses of Ontario (Extended Class)

Experience

Experience working with diverse and underserved populations
Experience in community health or primary care settings (preferred)

Interview Process

Stage 2: Credential verification and reference checks
