Job Description:Reporting to the Manager, Home TPN Clinic, the Home Total Parenteral Nutrition Patient Navigator is responsible for the assessment of patients to identify eligibility, learning needs, education planning and implementation, evaluation, coordination of care and follow-up of Home Total Parenteral Nutrition patients within Saskatchewan. The incumbent collaborates with multidisciplinary team members and liaises with health professionals and agencies within the SHA to facilitate high-quality care for the Home TPN patient.

Education

  • Bachelor of Science in Nursing (BScN)

Experience

  • One (1) year of experience in adult education or a teaching role
  • Requires experience in program/resource development related to needs assessment
  • Requires experience in planning, program design, implementation, and evaluation
  • Minimum 3 years’ experience with TPN and central venous access devices
