Job Description:Reporting to the Manager or designate, the Social Worker provides a range of screening, triage, assessment, treatment, and case coordination services to primarily adult community mental health and addiction population. The Social Worker provides services as a member of a multidisciplinary team. The incumbent will work collaboratively with community groups, agencies, and other professionals within the community.

Education

  • Bachelor of Social Work (BSW)

Knowledge and Abilities

  • Expert knowledge and advanced skill in a variety of treatment modalities
  • Committment to self evaluation, professional development and ongoing self care
  • Understanding social determinants of health for program development & delivery
  • Knowledge in local human service organizations, resources, referral procedures
  • Systems Theory/Family Systems Theory, strength-based model, crisis intervention
  • Ability to work with a variety of individuals presenting with complex issues
  • Knowledge in individual assessment and counselling skills
  • Ability to work cooperatively in an integrated interdisciplinary team
  • Knowledge in health promotion and prevention strategies
  • Committed to delivering on patient and family centred care
  • Ability to complete case management assessments, follow-up and treatment
  • Strong interpersonal skills
  • Demonstrates strong communication and organizational skills
