Job Description:The CDM Clinical Nurse Educator reports to the Manager Clinical Integration and works with an interdisciplinary team with a chronic disease prevention and management focus. The CDM Clinical Nurse Educator combines in-depth clinical expertise, with the principles of teaching and learning, in the provision of self-care management for clients, families, health professionals and students in group and individual settings.

Experience

3 years clinical experience with patients/families with chronic disease

Education

Bachelor of Science in Nursing (BScN)

Licenses

Valid Class 5 driver's license
Certified Diabetes Educator (CDE)

Other Education and Training

Basic Life Support Certificate (BLS)
Nursing diploma is acceptable
