Job Description:The Chronic Condition Nurse Educator (CCNE) integrates knowledge, skills and the principles of teaching and learning to support healthy lifestyle choices and chronic disease prevention, education and management services for clients, families, the general public and health professionals. The CCNE uses a consultative approach, accepted standards and guidelines and current research findings to provide support to clients and health care professionals as appropriate. The CCNE is an integral part of the Primary Health Care Team. The employee works relatively independently and is expected to make decisions based on sound assessments in accordance with policies of Population and Public Health Services, the health region, the College of Registered Nurses of Saskatchewan (CRNS) Standards of Practice and other relevant legislation and standards. The CCNE develops relationships with, and is supported by, specialty programs that manage chronic conditions.

Experience

  • Minimum of one (1) year nursing experience in a primary health setting
  • Experience in teaching adults, facilitation skills, and leading group activities
  • Experience in community needs assessment/development, health promotion
  • 3 yrs exp chronic disease/complex illness Edu. or chronic disease management required
  • Three (3) years nursing experience with patients/families with chronic disease

Education

  • Degree in Nursing with courses in supervision or degree in Edu./adult learning
  • Bachelor of Science in Nursing (BScN)

Licenses

  • Valid Class 5 driver's license

Other Education and Training

  • Nursing diploma is acceptable
  • Diabetes Education certificate (CDE) is acceptable
  • Basic Cardiac Life Support Certificate (BCLS)
  • Nephrology Certification is desired

  • Demonstrated knowledge of teaching abilities & group facilitation skills
  • Knowledge in population health promotion and its relationship to health status
  • Knowledge in Chronic Disease Management and behaviour change models
  • Experience working with patients/families with diabetes and renal disease.
  • Ability to work independently and with broader interdisciplinary teams
  • Knowledge in chronic disease prevention and management
  • Expert knowledge in relevant regulatory guidelines
  • Knowledge of program planning process
